Make WindowTextFont thread-safe in relevant areas and make texture-generation delayed. Measuring text no longer needs a GraphicsDevice.

Switch SignallingDebugWindow over to using new font code.

Fix SignallingDebugWindow line position so they're centred on start/end points.

Fix line-spacing in TextFlow window control.

Punch holes in the terrain where specified by MSTS RE. Without terrain transfer support, this can look a bit odd.

Visual keyboard map added to F1/Help.

Alt-F1 now dumps command list to "keyboard.txt".

Alt-F1 also dumps image of keyboard map to "keyboard.png".
